Filipinos who want to travel abroad may now apply for an International Vaccination Certificate or Yellow Card. Although it’s not yet mandated by local and international governments, it’s a document that’s recognized by the World Health Organization. It serves as a record booklet of a person’s immunization against infectious diseases and is issued by the Department of Health’s Bureau of Quarantine.
Anyone who is fully vaccinated can apply for an ICV, but priority is given to OFWs and those who need to fly out of the country. The Yellow Card will only be granted to those who received WHO-recognized vaccines such as Pfizer, Moderna, AstraZeneca, Johnson & Johnson, Sinovac, and Sinopharm.
An International Certification of Vaccine for COVID-19 costs P300 for the processing fee. There will be an additional P70 convenience fee, depending on your preferred payment method. Seniors get discounted services for P240.
NOTE: If you’re applying for a renewal of the certificate of vaccination or Yellow Card, show the BOQ personnel your old certificate before proceeding.
4. Certificate Printing. Claim your certificate of vaccination from the BOQ personnel.
The BOQ’s main office is located in Manila, but they also have sites in SM MOA, Batangas, Subic, and Cebu. Processing is strictly by appointment, so no walk-ins are allowed. Rescheduling after setting an appointment is not possible, and a no-show will forfeit your non-refundable payment.
